Include: (1) Current state documentation – how is process done today? Knowledge transfer needs? (2) Transition timeline – parallel run? Big-bang? Phased? (3) Knowledge transfer – how do you document and train vendor team? (4) Resource planning – how many internal staff support transition? (5) Risk mitigation – what could go wrong? Fallback plan? (6) Success criteria – how will you know transition is successful?

Include transition timeline and risk mitigation plan. Plan for setbacks.
